Our RN Case Managers collaborate with our clients to create personalized care plans that promote quality of life, independence, and safety in the home. The RN Case Managers coordinate with care providers and care team members to respond to our clients' changing needs.Essential Job Functions and Responsibilities:

• Completes initial and ongoing assessments of the client to determine homecare needs
  • Provides skilled nursing care to the client as needed
    • Creates the initial plan of care in conjunction with the client and family * Assesses and maintains the care plan with scheduled visits
      • Collaborates as a member of the care team to ensure continuity of care
        • Supervises care providers to ensure the client receives quality care
          • Completes, maintains, submits accurate and timely patient care notes relevant to the client's condition
            • Provides education to care providers, clients, and family members as appropriate
              • Assumes responsibility for personal growth and development through education and training opportunitiesPosition Qualifications:
                • Must have an unencumbered RN License in CO
                  • Must have at least 3 years of RN experience
                    • Current CPR certification
                      • Proficient in computer skills, EMR documentation, manage office products * Able to lift/transfer clients
                        • Must be a licensed driver with a vehicle in good working conditionBenefits:
