Create and initiate care plans through assessing the history of
current and previous illnesses* Spend quality one-on-one time with
patients and their families to support everyday needs* Care for
patients by providing initial and ongoing assessments of patient
and family needs* Record important health information including
temperature, pulse, respiration, and blood pressure* Administer
medications and treatments as prescribed by the physician* Provide
health care instructions to the patient as appropriate* Record
symptom management changes and outcomes* Complete, maintain, and
submit accurate and relevant clinical notes regarding the patients
condition and care given* And additional duties as
assigned*Qualifications:** An enthusiasm for providing the best
possible care to patients and their families* An RN license in good
standing* 1 year of experience in hospice or home health preferred*
A valid drivers license* Current CPR certification* Excellent
observation, verbal, and written communication skills* And
