Overview: The Physical Therapist (PT) is responsible for delivering
rehabilitative services to patients in a post-acute care
environment. This includes creating and executing treatment plans
based on physician referrals and adhering to regulatory guidelines.
The role also encompasses supervising therapy aides and students,
collaborating with interdisciplinary teams, and ensuring precise
documentation while following organizational policies and
professional standards. This position provides opportunities for

Summary The PT - Physical Therapist delivers direct rehabilitation
services in physical therapy to patients based on physician
referrals, ensuring compliance with all relevant laws, regulations,
and organizational standards. Education, Experience, and Licensure
Requirements Graduation from an accredited physical therapy
program, holding a BSPT, MSPT, or DPT (those graduating in 2003 or
later must possess an MSPT or DPT) Current registration/licensure
in the applicable state, with an obligation to maintain an active
license in good standing throughout employment. Preferred one (1)
year of experience in a post-acute care or similar setting CPR
certification required upon hire or to be obtained during
orientation; must remain current throughout employment. Specific

Implement and interpret departmental programs, goals, objectives,
policies, and procedures Exhibit proficiency in all competency
areas, including patient rights, safety, and sanitation Maintain
professional relationships with colleagues, vendors, and others
Ensure confidentiality of all proprietary and confidential
information Understand and adhere to organizational policies,
including those related to harassment and compliance Demonstrate
integrity and professionalism by following the organization’s Code
of Conduct and completing mandatory compliance training Essential
Functions Implement physical therapy treatment plans for patients
Set, evaluate, and adjust realistic, measurable, timely, and
functional goals Supervise and assess care provided by PTAs, Rehab
Aides, and PT and PTA students Document care accurately and in a
timely manner Utilize therapy software effectively and accurately
Provide excellent customer service and maintain a positive attitude
towards patients Assist in the safe evacuation of patients Exhibit
reliable and consistent attendance Employ reasoning skills and
sound judgment Communicate and collaborate effectively within an
interdisciplinary team Engage in various physical activities such
as sitting, standing, bending, lifting, pushing, pulling, stooping,
walking, reaching, and moving intermittently during work hours
