Job Title: Travel RN - Case Management Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum 2 years License: Active RN License Certifications: Current BLS Certification Must-Have: Knowledge of discharge planning Patient assessment skills Experience with patient family conferences Experience working with physicians on safe discharge plans Experience with skilled nursing facilities, home health, hospice, and long-term acute care hospitals Description: This is a travel position for a Registered Nurse specializing in Case Management. The unit focuses on high-risk discharge planning needs for patients. The case manager will round on patients and work collaboratively with medical doctors and nursing staff. The role involves arranging medication placement and transportation for patients. Ideal candidates will be detail-oriented and thrive in a multi-disciplinary team environment. Candidates should be able to manage stress effectively while remaining patient-focused. Experience with electronic medical records (EMR) systems is preferred. This position requires effective communication and collaboration skills. Candidates will work with a team of 2-3 case managers during day shifts.
